Job Description

Transportation Liaison- Leave Replacement Temporary The transportation liaison will operate and maintain vehicle(s) with a focus on safety, customer courtesy, and patient confidentiality. Must be able to respond to emergencies during transport promptly and arrange for appropriate dispatch of emergency responders. Will work closely with Health Center Leadership to facilitate the scheduling of transportation services at Damian Family Care Center's community organizations with a focus on identifying those who have restricted/depleted access to primary health care within the communities we serve. Responsibilities include coordinating, expediting and transporting, as applicable, residential clients between collaborative partners and Damian Family Care Centers, Inc. Supports the mission and philosophy of Damian Family Care Centers, Inc.

Temporary Leave Replacement Position Requirements Minimum requirements for this position are: High school diploma or GED. Must be 21 years of age or older; in good health and have at least three (3) years of driving experience. Must have a valid New York State Driver's license and possess a safe driving record. Able to read and interpret road signs in English. Ability to communicate effectively in English, both oral and written. Must be willing to submit to alcohol/drug screening, if required by employer Must be willing to work flexible hours when required, primary Mon-Fri 8 am to 4 pm Defensive Driving Certificate and First Aid training preferred. Bi-lingual in Spanish a plus.
