Job Description

Health Connect America, Inc. seeks a Mental Health Therapist to provide outpatient, in home, and community based services to children, adolescents, adults, and families. In this role, you will complete clinical assessments, develop individualized treatment plans, and deliver evidence based individual, family, and group therapy. You will collaborate closely with a multidisciplinary team, coordinate care with community partners and referral sources, maintain timely and accurate clinical documentation, and participate in supervision, training, and professional development while upholding a compassionate, client centered, culturally responsive approach to care. Responsibilities Provide individual, family, and group therapy to children, adolescents, adults, and families. Conduct comprehensive biopsychosocial assessments and diagnostic evaluations. Develop and update individualized, evidence-based treatment plans. Deliver in-home, in-office, and community-based therapeutic services. Provide crisis intervention, safety planning, and risk assessment as needed. Collaborate with a multidisciplinary team and coordinate care with community partners, schools, and agencies. Maintain timely, accurate clinical documentation in the EHR in accordance with regulatory and agency standards. Engage clients and families using a client-centered, trauma-informed, and culturally responsive approach. Participate in clinical supervision, case consultation, and ongoing training. Support outcome tracking, quality improvement, and compliance with ethical and legal standards.
