SENIOR CARE THERAPY

seeks a Bilingual Licensed Marriage and Family Therapist to provide compassionate psychotherapy to seniors and their families in long-term care, assisted living, and community settings. In this role, you will conduct assessments, develop evidence-based treatment plans, and deliver individual, couple, and family therapy in both on-site and telehealth formats. You'll collaborate closely with an interdisciplinary team and facility staff, contribute to care planning, and document in compliance with Medicare and regulatory standards. Responsibilities Provide individual, couple, and family therapy to seniors and their support systems in long-term care, assisted living, and community settings. Conduct comprehensive biopsychosocial assessments and formulate DSM-5 diagnoses as appropriate. Develop and implement evidence-based, person-centered treatment plans and update them regularly. Deliver services in both on-site and telehealth formats, ensuring accessibility and continuity of care. Collaborate with interdisciplinary teams and facility staff to coordinate care and participate in case conferences. Maintain timely, accurate clinical documentation in accordance with Medicare, insurance, and regulatory standards. Monitor treatment progress, adjust interventions, and provide crisis intervention as needed. Educate residents, families, and staff on mental health conditions, coping strategies, and community resources. Adhere to ethical and legal standards, including confidentiality and professional boundaries. Participate in ongoing training, supervision, and quality improvement initiatives to support clinical excellence.
