Join us today, and experience a career well lived. Outpatient Therapist The Therapist at Mentor Behavioral Health provides therapeutic interventions to youth, families and adults. Services are provided in homes, communities, office and virtually - whichever best meets the needs of individuals served. Responsibilities:
Conduct diagnostic evaluations. Provide therapy, family support, and crisis interventions to an individual, family and/or group using various treatment modalities. Provide mental health consultation to stakeholders and treatment team members. Develop individualized treatment plans in coordination with an interdisciplinary team Develop and coordinate discharge planning. Maintain timely and accurate health record documentation. Attend regular staff meetings, clinical meetings and one-on-one sessions with direct supervisor to process current status of cases. Provide 25 service delivery hours per week.
Qualifications:
Bachelor's, LSW + at least 3 years' experience with children /families Master's Degree in human services + LPC, MFT We have meaningful work for you - come join our team - Apply Today!
