Description:

The Care Coach acts as a liaison between patients of the Baptist Health South Florida, their relatives and Baptist Health departments. They assist patients and their families by providing emotional support, ensuring timely consultations and diagNstic appointments, providing educational information regarding diagNsis, treatment, community resources, referrals and any other assistance needed in order to ensure patients are receiving the maximum benefits from medical care. Estimated salary range for this position is $50457.98 - $65595.37 / year depending on experience.

Additional Qualifications:

Bachelor degree desired. Experience in Women's Breast health required for Breast disease site. Individuals with experience in managing chronic illness including Oncology care in the following disease sites will be considered: Neurology, Dermatology, Breast, Thoracic, Genitourinary, Gastric, Gynecology, Head and Neck, EndocriNlogy, Hematology to include solid and liquid tumors. Nursing, Social Work, Imaging techNlogy or related health professional degree desirable.
