Flexible to meet the needs of the caregivers Position Summary The Care Coach position has been created to lend support to caregivers that are caring for a family member with complicated needs. As a care coach, you will go to the homes of our caregivers and help them find resources, offer support, and build good will. The goal of a Care Coach is to make 1 visit per quarter and build support for the caregiver. Encouraging self-care and helping the caregivers set goals for themselves. This position will support Cluster J (Hancock, Shelby, Rush, Fayette, Union, Franklin, Decatur, Monroe, Brown, Bartholomew, Lawrence, Jackson, Jennings, Ripley, Dearborn, Jefferson, Switzerland and Ohio counties). Responsibilities/Job Duties Make home visits as required per state guidelines to ensure compliance. Maintain records of visits Communicate with the Care Coach Supervisor Travel within the assigned territory to see caregivers in their homes. Coordinate visit times with the caregiver to ensure they are scheduled at mutually convenient times and commit to honoring scheduled appointments—cancelling only in true emergencies. Document notes from visit in required apps on computer/ phone. Attend monthly team meetings (virtually). Perform other duties as assigned. Required Qualifications- High school diploma or equivalent Strong understanding of the emotional and physical demands placed on caregivers.
