The ideal candidate will possess a strong background in public health, social work, or nursing, complemented by experience working with diverse populations and underserved communities. As a Wellness Coordinator, you will design, implement, and evaluate programs that promote healthy lifestyles, provide health education, and foster community engagement. Your role will be pivotal in enhancing the overall wellness of individuals through personalized support, outreach efforts, and collaborative community partnerships. Duties Develop and coordinate comprehensive wellness programs tailored to the needs of diverse populations, including underserved communities. Conduct health education sessions on topics such as nutrition, physical activity, mental health, and chronic disease prevention. Engage in community outreach activities to raise awareness about available health services and resources. Provide client-centered support using motivational interviewing techniques to encourage healthy behavior changes. Collaborate with healthcare providers, social workers, and community organizations to facilitate access to services and resources. Maintain accurate documentation of client interactions and program outcomes using EMR/EHR systems. Organize public speaking engagements and workshops to promote health literacy and community involvement. Collect data for program evaluation and continuous improvement efforts while ensuring compliance with public health laws and policies. Requirements Bachelor's degree in Nursing, Public Health, Social Work, or related field; advanced degrees are a plus. Proven experience working with individuals with disabilities or from underserved communities. Strong knowledge of public health law policies and community health best practices. Proficiency in Microsoft Office Suite for report writing, data collection, and presentation purposes. Excellent communication skills in both written and verbal formats; bilingual abilities are highly desirable. Experience developing and implementing public health programs with a focus on community engagement. Familiarity with EMR/EHR systems for documentation purposes. Knowledge of basic life support (BLS) certification or willingness to obtain certification upon hire. Ability to work effectively with culturally diverse populations while demonstrating cultural sensitivity and inclusivity.
