We're looking for a Wellness Case Manager to guide patients through their wellness journey, from their first conversation with our clinic through ongoing treatment plan engagement. This role is built for someone who has spent their career at the intersection of health, wellness, and sales, and who genuinely enjoys both. You'll work closely with providers and clinic leadership, helping patients stay engaged, informed, and connected to the services that support their health goals. What You'll Be Doing Building strong, trust-based relationships with patients from their very first interaction Guiding patients through NIVA's wellness and therapy offerings, helping them understand what fits their goals Supporting patient engagement, retention, and wellness treatment plan participation Coordinating follow-ups, scheduling, and patient communications Creating a seamless, elevated patient experience at every touchpoint Tracking personal KPIs tied to engagement, follow-up, and patient enrollment in new services Supporting new wellness initiatives and service launches within the clinic Partnering closely with providers and clinic staff in a collaborative environment
