Job Description
Overview We are seeking a dedicated and knowledgeable Diabetes Educator to join our healthcare team. The ideal candidate will possess a strong background in clinical nutrition, chronic disease management, and patient education, with a focus on empowering individuals to manage their diabetes effectively. As a Diabetes Educator, you will play a vital role in developing personalized care plans, providing comprehensive nutrition counseling, and collaborating closely with healthcare professionals to optimize patient outcomes. This position offers an opportunity to make a meaningful impact on patients' lives through education, support, and expert guidance. Responsibilities Conduct thorough patient assessments to evaluate individual health status, nutritional needs, and diabetes management goals. Develop and implement tailored care plans that incorporate medical nutrition therapy (MNT), lifestyle modifications, and medication adherence strategies. Provide detailed nutrition counseling and health coaching to support patients in managing their diabetes and related chronic conditions. Utilize motivational interviewing techniques to encourage behavioral changes and foster patient engagement. Monitor patient progress through electronic health records (EHR) systems, documenting interventions and adjusting plans as necessary. Educate patients on topics such as blood glucose monitoring, insulin administration, carbohydrate counting, weight management, and dietary restrictions including tube feeding when applicable. Stay current with advancements in clinical nutrition, physiology knowledge, and diabetes management protocols to provide evidence-based care. Qualifications Bachelor's degree in dietetics, nutrition, or a related field; advanced certifications such as Certified Diabetes Educator (CDE) are highly preferred. Proven experience working with individuals managing diabetes or other chronic diseases in hospital or long-term care settings. Strong knowledge of physiology, medical nutrition therapy (MNT), and dietary interventions specific to diabetes management. Demonstrated expertise in health coaching, motivational interviewing, and client communication skills. Experience working with patients requiring specialized nutritional support such as tube feeding or dietary modifications for geriatrics or acute care populations. Knowledge of nutritional analysis tools and diagnostic evaluation methods to assess nutritional status effectively.
