The position provides education, technical assistance, practice coaching, and quality improvement support related to diabetes, cardiovascular disease, arthritis, and other priority chronic conditions. Responsibilities include engaging healthcare and public health partners, assessing organizational and clinical needs, supporting practice and systems change, using data to identify improvement opportunities and monitor outcomes, and developing and delivering education and resources that advance chronic disease prevention and management. The position requires regular travel throughout Oklahoma to provide onsite consultation, education, technical assistance, and stakeholder engagement.
RESPONSIBILITIES AND DUTIES
- Collaborate with multidisciplinary OFMQ team members to identify and/or develop efficient and effective improvement interventions, strategies, and tools. Periodically review pertinent literature and share, as appropriate, with providers, stakeholder organizations, and OFMQ associates.
- Regularly review intervention materials for quality improvement (QI) projects and assess for appropriate and effective use.
- Provide education, consultation, coaching, and technical assistance to healthcare organizations, public health entities, and community partners to support evidence-based chronic disease prevention and management initiatives, with emphasis on diabetes, cardiovascular disease, and arthritis.
- Recruit and engage physician practices, Rural Health Clinics, Federally Qualified Health Centers, hospitals, health systems, public health organizations, and other healthcare partners to participate in quality improvement and chronic disease prevention initiatives.
- Coordinate and conduct onsite and virtual visits for education, training, consultation, organizational assessment, and identification and sharing of successful intervention strategies.
- Conduct assessments to identify clinical workflow, patient care, data, education, and organizational opportunities for improvement and collaborate with participating organizations to address barriers to achieving QI goals.
- Assist healthcare organizations in implementing evidence-based clinical and quality improvement strategies, including screening, early identification, referral, treatment, patient education, self-management support, and follow-up for chronic conditions.
- Assist participating organizations in developing, implementing, monitoring, and evaluating quality improvement plans using methodologies such as Plan-Do-Study-Act (PDSA), rapid-cycle improvement, workflow mapping, root cause analysis, and other appropriate QI methods.
- Work with healthcare organizations to identify, collect, interpret, and use clinical quality measures and other performance data to identify care gaps, establish improvement priorities, monitor progress, and evaluate outcomes.
- Assist healthcare organizations in using EHR reports, patient registries, dashboards, and population health management tools to identify priority populations and support chronic disease prevention and management activities.
- Promote implementation of evidence-based guidelines, protocols, clinical decision support tools, standing orders, and other systems-level interventions that improve chronic disease prevention and management.
- Support development and implementation of systems that strengthen referrals between clinical care, public health, and community-based programs, including evidence-based lifestyle change, diabetes prevention, physical activity, nutrition, and chronic disease self-management programs.
- Build and maintain collaborative relationships among healthcare providers, hospitals, public health agencies, community-based organizations, and other stakeholders to strengthen clinical-community linkages and chronic disease prevention infrastructure.
- Regularly research national education and chronic disease prevention resources and use and/or adapt materials to meet the needs of providers and other partners in Oklahoma.
- Collaborate with OFMQ interdisciplinary team members and assist in the design, education, and implementation of regional and/or statewide conferences, learning collaboratives, Communities of Practice, and other peer-learning activities.
- Act as speaker/faculty for educational presentations and conferences as needed. Develop and maintain presentations, webinars, toolkits, technical assistance resources, and other educational materials.
- Observe provider quality implementation progress and assist organizations in developing system changes, performing self-measurement, interpreting data, and adjusting improvement plans to progress toward goals.
- Assist in the development of measurement tools to track progress toward goals and coordinate the development and dissemination of provider/collaborative feedback materials such as project data reports, best practices, and implementation resources.
- Monitor current clinical guidelines, evidence-based practices, national and state chronic disease priorities, and emerging healthcare quality initiatives and translate relevant information into practical resources and interventions.
- Document technical assistance, partner engagement, practice changes, implementation activities, barriers, outcomes, and other information necessary to meet organizational, contractual, grant, and program reporting requirements.
- Participate in program evaluation and continuous quality improvement activities, including analysis of implementation progress, identification of barriers and successful practices, and development of recommendations for program improvement.
- Provide individualized technical assistance and practice coaching that recognizes the unique workforce, technology, financial, and operational challenges of rural and underserved healthcare organizations and assists organizations in adapting evidence-based interventions to local resources and patient populations.
- Represent OFMQ at meetings, conferences, coalitions, workgroups, and other stakeholder activities related to chronic disease prevention, healthcare quality, population health, and public health.
- Collaborate with Program Director, Personnel Manager, managers, and other staff to provide ongoing educational opportunities for OFMQ employees as needed.
- Work with OFMQ leadership in exploring development of alternative partnerships with Oklahoma organizations and businesses interested in evidence-based healthcare quality and chronic disease prevention programs.
- Travel regularly throughout Oklahoma, including periodic overnight travel, to conduct site visits, provide onsite technical assistance and education, participate in stakeholder meetings, and support program implementation.

MINIMUM QUALIFICATIONS REQUIRED
Education and Experience:
Bachelor's degree in health education, public health, nursing, nutrition/dietetics, healthcare administration, health sciences, or a related field from an accredited college or university, and a minimum of three to five years of relevant experience in healthcare quality improvement, chronic disease prevention, population health, public health, clinical practice, or healthcare education. An equivalent combination of education and experience may be considered.
Preferred Qualifications:
Master's degree in public health, health education, nursing, nutrition, healthcare administration, or a related discipline preferred. CHES/MCHES, CPH, CPHQ, RN, RD/RDN, CDCES, or other relevant healthcare, public health, or quality improvement credential preferred. Experience providing technical assistance to healthcare organizations, implementing chronic disease initiatives, working with clinical quality measures, facilitating quality improvement activities, and engaging rural or underserved healthcare organizations is highly desirable.
Knowledge and Skills:
- Knowledge of curriculum development, project management, research principles and practices, adult learning principles, quality improvement, chronic care management, preventive care, population health, and health promotion.
- Knowledge of evidence-based prevention and management strategies for diabetes, hypertension, cardiovascular disease, arthritis, and related chronic conditions.
- Knowledge of clinical quality measures, performance measurement, and improvement methodologies, including PDSA cycles, workflow analysis, root cause analysis, process mapping, and practice transformation.
- Ability to interpret clinical and population-level data and translate findings into actionable improvement strategies.
- Familiarity with EHRs, patient registries, population health tools, dashboards, and electronic clinical quality measurement.
- Understanding of clinical-community linkages and referral systems connecting patients with evidence-based community programs and services.
- Knowledge of healthcare delivery across physician practices, Rural Health Clinics, Federally Qualified Health Centers, hospitals, health systems, and public health settings.
- Advanced communication, presentation, facilitation, coaching, and interpersonal skills to engage physicians, nurses, administrators, quality staff, public health professionals, and community partners.
- Excellent writing skills with the ability to produce professional project documents, educational materials, technical assistance resources, and multimedia presentations.
- Ability to work independently, show initiative, manage multiple provider organizations or project sites simultaneously, and complete tasks and projects in a timely manner.
- Excellent skills in prioritizing, planning, organizing workflow, maintaining documentation, and meeting contractual or grant deliverables.
- Ability to maintain excellent customer and partner relationships and effectively communicate project goals and expectations.
MENTAL AND PHYSICAL ABILITIES
