Full-Time We are seeking a compassionate and community-focused Community Health Worker / Program Coordinator / Health Educator to support individuals and populations who may experience barriers to healthcare access and health disparities. This position will serve as a connection between individuals, healthcare resources, and community organizations. The ideal candidate will be comfortable working directly within the community, building relationships with community partners, providing health education, and helping individuals navigate the healthcare system and access appropriate resources. Responsibilities Assist individuals and populations at risk for health disparities in navigating the healthcare system and accessing appropriate services and resources. Work closely with community-based organizations and other community partners to connect individuals with healthcare and social services. Provide health education, health information, and resource guidance to individuals and community members. Provide social support and informal counseling while maintaining appropriate professional boundaries. Advocate for individuals who have healthcare or social service needs. Help identify barriers to healthcare access and connect individuals with appropriate resources. Build and maintain positive relationships with community partners and organizations. Participate in outreach activities and engage with individuals in community and public settings. Maintain strict confidentiality of patient and participant information. Document interactions, services, referrals, and other required information accurately. Perform other duties related to community health outreach and program coordination as needed. Qualifications High school diploma or equivalent required. At least 1 year of experience working in a community-based setting , including paid or volunteer experience with local community-based organizations.

Strongly preferred:

Certified Community Health Worker (CCHW) certification in South Carolina. Strong communication and interpersonal skills. Ability to demonstrate empathy, compassion, and cultural sensitivity when working with individuals from diverse backgrounds. Ability to build trust and establish positive relationships within the community. Strong organizational skills and the ability to work independently. Ability to maintain strict patient confidentiality and handle sensitive information appropriately. Comfortable working in community and public settings and traveling throughout the local community as needed. Must be willing to spend approximately 80% of the time working in the community/public settings and 20% in an office environment.
