Mercy is seeking a dedicated Clinical Coordinator to support high-quality, patient-centered care across our multidisciplinary teams. In this role, you will coordinate patient care plans, streamline clinical workflows, and serve as a key liaison between patients, families, and providers. Responsibilities include managing schedules, ensuring accurate EHR documentation, supporting regulatory compliance, and driving quality improvement initiatives. You will collaborate closely with nurses, physicians, and allied health professionals to enhance patient outcomes. Responsibilities Coordinate and monitor patient care plans across multidisciplinary teams. Serve as primary liaison between patients, families, and clinical staff. Ensure accurate and timely clinical documentation within EHR systems. Support compliance with regulatory and quality standards. Assist with scheduling, resource allocation, and patient flow optimization. Participate in quality improvement and population health initiatives. Educate patients and families on care plans, treatments, and follow-up. Collaborate with leadership to identify process improvements in clinical operations.
