The Social Services Coordinator serves as an advocate for our residents by assisting with admissions, discharge planning, resident and family concerns, community resources, and ensuring residents achieve their highest practicable level of psychosocial well-being. We also encourage Licensed Vocational Nurses (LVNs) with strong care coordination or case management experience to apply. Responsibilities Coordinate resident admissions and discharges. Assist residents and families with adjustment to long-term care. Conduct psychosocial assessments and maintain appropriate documentation. Coordinate discharge planning with, physicians, therapy, nursing staff, families, and community agencies. Assist residents and families with Medicare, Medi-Cal, insurance, and community resource referrals. Participate in care plan meetings and interdisciplinary team meetings. Assist with resident rights, grievances, abuse reporting, and resident advocacy. Help facilitate family meetings and communication between residents, families, and the interdisciplinary team. Maintain accurate documentation in accordance with state and federal regulations. Work closely with Nursing, Admissions, Activities, Rehabilitation, Dietary, and Administration. Perform other duties as assigned. Qualifications Previous experience in a skilled nursing facility, hospital, rehabilitation center, hospice, social services, case management, or healthcare setting preferred. Experience with discharge planning, resident advocacy, or care coordination is preferred. Knowledge of Medicare, Medi-Cal, and long-term care regulations is a plus. Excellent communication, organizational, and customer service skills. Ability to manage multiple priorities while maintaining professionalism and compassion. Licensed Vocational Nurses (LVNs) with strong organizational and care coordination skills are encouraged to apply.
