Solomon Medical Inc. is a leading post-acute care practice dedicated to elevating care standards in Skilled Nursing Facilities (SNFs) across Central and Northern California. We're seeking a Medical Coordinator (MC) to play a key role in supporting our highly-regarded team of Physicians and Nurse Practitioners in Santa Clara County. ESSENTIAL DUTIES AND RESPONSIBILITIES
Greet/serve as the first point of contact for patients. Obtain daily nursing reports regarding acute patients and RX refills. Manage and schedule provider visits for MD/NP on a daily basis as well as scheduling out visits based on Medicare requirements and Solomon Medical guidelines. Input new patient's data including demographic and insurance information into the EMR. Initiate administrative tasks including implementing new facilities, onboarding providers, and creating monthly schedules. Act as a liaison between the Solomon Medical billing company and facilities for insurance inquiries. Act as a liaison between the facility administrator/nursing staff and our Solomon Medical clinicians helping to ensure smooth operations. Obtain all reports including DME, Insurance verification, prior authorization, labs, radiology to be reviewed and signed by the providers.
New admissions:
obtain DC summary and create new patient encounter within EMR. Transcribe key DC summary information. This includes hospital course, updated meds, allergies, past medical history, past surgical history, social history. Maintain business professionalism and etiquette as well as adhering to HIPAA standards as it relates to patient data.

EDUCATION AND EXPERIENCE
Bachelor's degree in healthcare administration, nursing, or related field preferred. Previous experience in a SNF or medical/healthcare setting is advantageous. Familiarity with electronic medical health care record systems and medical insurance. A basic understanding of patient care and medical terminology
