We're looking for an organized, friendly team member to keep patients and their families connected and the office running smoothly. About the Role This is a mostly office-based role focused on patient and family outreach and front-office support. You'll work closely with our attending physician and practice owner and occasionally help with tasks he needs handled. What You'll Do Patient & Family Outreach Call patients to schedule visits, follow-ups, and annual wellness exams Confirm appointments and send reminders Follow up with patients who missed visits or are due for care Communicate with patients' families and caregivers about patient status, follow-up plans, and referrals Keep families informed on next steps and answer non-clinical questions Coordinate with pharmacies, labs, specialists, and home health agencies Office Support Answer phones, return messages, and respond to patient questions Enter and update patient information in the EMR Handle faxes, referrals, records requests, and filing Keep the physician's schedule organized and charts ready for visits Other Duties Assist the physician-owner with occasional administrative or business tasks as needed What We're Looking For High school diploma or equivalent 1+ year in a medical office, call center, or customer service role (preferred but, not required) Clear, professional phone skills and a patient-first attitude Compassionate, clear communicator with patients, families, and caregivers Comfortable with computers; EMR experience a plus Organized, reliable, and able to manage a call list independently Flexible and willing to pitch in where needed Discreet with confidential patient and business information Understanding of HIPAA and patient privacy (training provided) Bilingual English/Spanish a plus Knowledge of Medicare/Medicare Advantage a plus Schedule [Monday-Friday, 8:00 AM-5:00 PM]
