Patient Outreach Coordinator Merced Faculty Associates Merced, CA Job Details Full-time From $23 an hour 19 hours ago Benefits Health insurance Dental insurance 401(k) Paid time off Vision insurance Qualifications Computer operation Customer communication HIPAA High school diploma or GED Data management Managing patient records Full Job Description Job Summary The Patient Outreach Coordinator plays a crucial role in ensuring the success of medical professionals' treatment plans by working collaboratively with patients, healthcare providers, and practice teams to deliver the best possible care. This position primarily involves managing clinical data using the Electronic Medical Record (EMR) system and conducting outreach to patients and healthcare facilities to coordinate care. These efforts are guided by quality management reports, managed care reports, and outstanding clinical orders, all aimed at promoting optimal patient outcomes. Duties Evaluate and analyze patient information from Electronic Medical Records (EMR), chart reviews, and other sources to support monitoring and ensuring adherence to prescribed care plans. Support the care team by tracking data, conducting patient outreach, and accurately documenting patient communications. Work collaboratively with patients and healthcare providers to identify and implement opportunities to enhance care. Coordinate patient care and related services to achieve established quality metrics. Uphold patient confidentiality by respecting privacy rights and strictly adhering to HIPAA guidelines and regulations. Engage in Quality Improvement (QI), Quality Assurance (QA), and Continuous Quality Improvement (CQI) initiatives as needed. Perform additional tasks as assigned by management. Qualifications High school diploma or GED required. Previous experience as a receptionist and/or medical assistant is preferred. Strong communication skills to effectively engage with staff, medical professionals, and patients. Commitment to providing excellent customer service. Excellent organizational and interpersonal abilities. Basic computer proficiency and demonstrated ability to use common software applications. Pay:
From $23.00 per hour Expected hours: 40.0 per week
