They are an expert in phone communication specifically communicating with patients via telephones in a friendly, professional, and welcoming manner. Their primary duty will be to proactively communicate with patients to facilitate adherence to Unicare Community Health Center measures. They are integral part of the healthcare delivery team, demonstrate the Unicare CHC mission. Assist and support the Front Office Lead and Health Center Manager in accordance with current policy and procedure, applicable federal, state, and local standards, guidelines, and regulations. Essential Functions for Patient Care Coordinator 1. Display a positive attitude while helping patients access to health care and overcome barriers. 2. Contact new enrollees of all age groups under various health plans to set up initial appointments. 3. Coordinate special screening programs sponsored by health plan. 4. Schedule and confirm appointments for multiple providers. 5. Follow-up with appointment cancelations and no shows. 6. Back up the phone center during times of high call volume 7. Screen incoming and routine calls; take detailed messages 8. Confirm insurance eligibility and assist patient with insurance verification. 9. Improve the patient relationship with Unicare Community Health Center. 10. Assist medical providers in retrieving Healthcare Effectiveness Data and Information (HEDIS) to improve health care quality. 11. Identify and communicate significant changes in patient's status (potential or actual adverse patient occurrences) to the medical provider. 12. Increase utilization of preventive care. 13. Promote timely access to appropriate care. 14. Increase continuity of care by managing relationships with tertiary care providers, transitions-in-care, and referrals. 15. Provide culturally sensitive services to patients from different cultures. 16. Identify and utilize cultural and community resources. 17. Establish and maintain relationships with identified service providers. 18. Triage the patient's needs to get the appropriate care for the patient within the appropriate timeframe. Professional Requirements Adhere to dress code, appearance is neat and clean. Handle confidential and protected health information (PHI); understand of the importance of confidentiality and non-disclosure according to the general standards set forth by HIPAA. Maintain professionalism in all interactions with patients, family members, providers, office staff, members of the community and related outside agencies. Attend general and medical staff meetings. Maintain cleanliness and organization of the office area assigned to you. Proficiently use Microsoft Office Suite and electronic medical/health records. Typing a minimum of 35 WPM. Understand and constantly apply universal precautions if assisting in patient care. Report to work on time and as scheduled. Maintain regulatory requirements, including all state, federal and local regulations. Represent the organization in a positive and professional manner at all times. Comply with all organizational policies and standards regarding ethical business practices. Communicate the mission, ethics and goals of the organization. Participate in performance improvement and continuous quality improvement activities. Performs other related duties as may be assigned by the Referral Department Supervisor and/or Health Center Manager. Qualifications
  • Bilingual English/Spanish required
  • Minimum of one-year experience working in community health setting required
  • High School Diploma/GED required Experience Working in community or health settings Ability to define problems, collect data, establish facts, and draw valid conclusions Ability to interpret and extensive variety of data Meets and interact with patients, necessary to adjust or change priorities, handle multiple tasks, etc. Proficiency in Microsoft Office Suite
  • Ability to use electronic medical/health records
  • Typing speed minimum of 35 words per minute Strong customer service focus Effective verbal and written communication skills Speak, read, and write English and Spanish fluently Team-oriented Organized and able to manage competing priorities Good judgment Able to problem-solve Able follow through with delegated tasks and accountability
Training, Education, and Licensure:
Medical Assistant Certification Preferred BLS certificate Required Physical Requirements and Environmental Conditions Pushing and pulling objects up to 25 lbs. Frequent wrist , hand, and finger dexterity to perform fine motor function. Full range of body motion including twisting body, pushing, pulling Position requires light to moderate work with 25 lb. maximum weight to lift and carry. Position requires reaching, sitting down for long periods of time, walking, bending, stooping, and handling objects with hands and/or fingers, talking and/or hearing, and seeing. WORK SCHEDULE
Monday -
Friday:
Starting time at 9:00 am and ending time at 6:00 pm. Required to work occasional Saturdays, as needed. May travel to all Unicare Community Health Center clinics to work or attend meetings as needed.
