  • Ambulatory Care. This role will report to a Supervisor

Put Patients First Empower Entrepreneurial Provider and Care Teams Operate with Integrity & Excellence Be Innovative Work As One Team What You'll Do Responsible for member/provider outreach and notification (telephonic and/or written) utilizing department templates Responsible for assisting ACM nurses with member assessment and communication, which includes but is not limited to eligibility verification, requesting medical records, creating case, and documenting care activity Participate in all health plan and department meetings and trainings as applicable Assist with health plan audit preparation Ensures maximum utilization of resources Exceed standard benchmarks with performance objectives Provide ongoing support and mentorship to ACM Coordinator I and other team members as needed. Take the lead in delegated projects and assignments Assist in the development of training for new ACM coordinators Perform other duties, projects and actions as assigned Qualifications High school diploma OR GED Experience using Microsoft applications such as Word, Excel and Outlook You're great for this role if: Previous experience with SNP Case Management Able to speak and understand Mandarin or Cantonese Experience working in a MSO/IPA/Health Plan Environmental Job Requirements and Working Conditions Our organization follows a hybrid work structure where the expectation is to work both in office and at home on a weekly basis. The office is located at 600 City Parkway West 10th Floor, Orange, CA 92868. The target pay range for this role is $19
  • $22 per hour.
