JOB OPPORTUNITY
The Community Programs Coordinator is a member of the Community Programs team focused on providing administrative and clinical support in service of patients and community members receiving services and care in WCHC community programs. Position is a full-time, on-site role located at our health center in Sebastopol, CA. Travel to other health center sites required.DUTIES Administrative support includes:
Development and distribution of educational and promotional materials, Inventory and update of community resources Tracking clinical measures and outcomes through data entry and analysis Implementation of quality improvement initiatives Assistance with the scheduling and administration of programmatic meetings and community events, such as mobile clinic events Planning and advertisement of community meetings and mobile clinical events. Other duties as assigned.Clinical support includes:
Works with clinical teams to assist with care management and resource navigation for complex care patients including supporting barrier reduction, health and mental health coaching, and system coordination. Assistance with scheduling and confirmation of clinical appointments. Provide check-ins between visits to establish and support patient goals. Collaborate with clinical staff to facilitate patient groups. Facilitation of risk mitigation services. Other duties as assigned.EXPERIENCE
Certificate of completion from Community Health Worker Program or equivalent, or willingness to enroll in training as condition of employment 1-2 years' experience working within a healthcare or community setting. Knowledge of community resources and networking ability. Must have an acute attention to detail and follow through on commitments. Strong organizational skills and the ability to multitask effectively in a busy environment. Expected to have strong interpersonal skills and can understand and follow written/verbal instructions. Ability to work independently with little or no supervision. Excellent interpersonal skills to interact positively with patients and staff alike. Proficiency in using office software and electronic health record systems is beneficial.PHYSICAL REQUIREMENTS
Ability to move freely about the clinic/office and carry light loads (up to 25lbs). Ability to sit for extended periods of time and stand for minimum periods of one hour. Must be able to perform the essential duties with or without accommodations. Basic office and equipment safety knowledge and practice.
