Job Summary :
Under the supervision of the Wellness Center Manager and in conjunction with the Clinical Groups Coordinator, the Wellness Center Coordinator is responsible for implementing office systems to support patients within the West County Health Center Wellness Center. This position promotes optimal patient experience by assisting patients, providers, support staff, and partner agencies with their administrative needs, and managing patient information within the electronic health record including referral and prior authorization coordination. The Wellness Center is unique in offering integrative health services including Acupuncture clinics, Osteopathic Manipulation visits, group medical visits, and individual consultations , providing a diverse and engaging work environment. Schedule is Monday
  • Friday, 8:30am
  • 5:00pm or 8:45am
  • 5:15pm.
Some travel to Guerneville, CA is required.
DUTIES:
(may not include all duties assigned) Monitors scheduling, office flow, and coordination of all Wellness Services. Develops and implements workflows and systems that maximize efficiency and promote an excellent patient experience. Seeks out opportunities to assist other team members to collaboratively meet the needs of patients and Care Teams. Assists in the adoption and implementation of new technologies and activities that improve patient care and clinic efficiencies. Assists with clinical data outcome reporting and actively participates in quality improvement projects related to Wellness Services. Participates in activities related to addressing the social determinants of a patient's health. Works with Site Medical Director, Clinical Director, and Clinical Groups Coordinator to maintain Wellness Services education materials, handouts, etc. Responsible for flyer creation, distribution, marketing, and presentations for Wellness Services.
Attends regular meetings with:
Wellness Front Office, Care Teams, and Site Management, plus WCHC All Staff and Department Meetings at Primary Care Sites. In conjunction with Wellness Center Manager, tracks Patient-Provider Consult visits and Group Medical Visit attendance. Maintains professional appearance of waiting room and all patient areas. Maintains up to date information on availability of specialty care in the county for patients who need care beyond Wellness availability. Specific to Group Medical Visits (GMV) Serves as a resource in creating, establishing, and maintaining agency GMV's. Hosts training for front office and clinical staff, including MAs and RNs at all sites. In conjunction with Wellness Center Manager, assesses sustainability of Wellness GMV's. Participates in Wellness and Agency GMVs as needed. Attends training and conferences to maintain and evolve the agency's GMV program. Schedules and leads Care Team meetings for all Wellness GMV's, records meeting minutes, and distributes among Wellness Center staff. Attends regular meetings with WCHC Behavioral Health Leaders to review current groups involving behavioral health providers and to plan future groups based on patient needs and emerging trends. Assists with incident reports and keeping GMVs a safe place for patients, providers, and content experts, assists with warm handoffs to BH as needed. Follows up with patients post-group regarding satisfaction, group participation, and quality improvement. Referrals Serves as a resource and coordinates timely referrals for all Wellness Services from all WCHC providers, Care Teams, and agencies outside of WCHC. Using the EHR system, tracks successful completion of referral process for all Wellness Services. Works with the Data team to ensure correct tracking for provider incentive for primary care providers and billing providers within the Wellness program including LAcs. Actively tracks referral waitlist times, and works with Clinical Groups Coordinator and Site Medical Director to adjust referral triage protocols and service offerings as needed. Connects patients on long waitlists for 1:1 Wellness Consults with appropriate Wellness GMV's. Assists with program planning and agency policy development regarding Wellness referrals as appropriate. Treatment Authorization Requests (TAR) Confirms insurance eligibility and demographic information, prepare/submit TARs, and track/complete the authorization process for Acupuncture treatments offered by the Wellness Center. Maintains regular communication with health plan representatives both via telephone and in written form. Works in collaboration with Acupuncture Department Manager, Collaborative Medical Providers, and Licensed Acupuncturists to obtain proper diagnosis codes and billing information for TAR approval. Tracks successful completion of TAR process for all Partnership Health Plan patients receiving Acupuncture. Maintains TAR lists and works directly with Patient Services Representatives to ensure proper scheduling.
Experience:
Bilingual Spanish/English desirable. Experience in Health Education, degree in Health Education, and/or experience and interest with Group Medical Visits. Experience in an outpatient setting or working with a health clinic desirable. Demonstrated commitment and skills to provide age-appropriate care to a diverse population Familiarity with medical terminology and billing. Knowledge and experience of medical front office duties. Computer literacy and familiarity with office machines such as fax, copier, calculator. Good communications skills Experience with Excel, Word, Zoom, Teams, etc.
Physical Requirements:
Ability to bend and move arms with sufficient range of motion to complete patient related requirements, hearing required for patient assessments, may need to assist patients onto exam table, able to lift 30 lbs., bend, kneel and twist as needed. Prolonged standing and occasional sitting required. Must be able to perform the essential duties with or without accommodations. Basic office and equipment safety knowledge and practice.
