Position Summary The Inpatient Wound/Ostomy Care Coordinator provides specialized clinical expertise in the assessment, treatment, and management of patients with complex wounds, pressure injuries, ostomies, and skin integrity concerns. The Coordinator serves as a clinical resource to nursing and interdisciplinary teams and promotes evidence-based practices to improve patient outcomes and reduce hospital-acquired pressure injuries. Key Responsibilities Assess and manage patients with complex wounds, pressure injuries, and ostomies. Develop, implement, and evaluate individualized wound and ostomy care plans. Serve as a clinical resource and consultant to nurses, providers, and interdisciplinary teams. Recommend appropriate wound care products, dressings, ostomy supplies, and support surfaces. Provide patient and caregiver education regarding wound and ostomy care. Conduct wound care rounds and support pressure injury prevention initiatives. Provide staff education, training, and competency development related to wound and skin care. Monitor wound-related quality metrics and participate in performance improvement initiatives. Collaborate with Nursing, Infection Prevention, Quality, Nutrition, Case Management, and other departments to improve patient outcomes. Support development and implementation of wound and ostomy policies, protocols, and best practices. Promote continuity of wound and ostomy care during discharge and transitions of care. Qualifications Current Connecticut Registered Nurse (RN) license. Associate Degree in Nursing required; BSN preferred. Minimum of 3 years of clinical nursing experience, preferably in wound care, ostomy care, medical-surgical, critical care, or a related specialty. Strong clinical assessment, communication, organizational, and critical-thinking skills. Experience with wound assessment, pressure injury prevention, and ostomy management.
