Seeking top talent, dedicated full time Home Health RN Patient Care Manager for a national leader in personal care, medical and non medical home health, hospice, and palliative care services needed for the Ormond Beach area of Florida working Monday-Friday in office! The Patient Care Manager will supervise clinical team as well as coordinate, plan, and monitor patient care. Must be a current RN licensed in the state of FL with previous experience in home care setting with 2 years management or supervisory experience preferred.
