Patient Care Coordinator
REPORTS TO
Front Office Supervisor, Director of Operations
JOB SUMMARY
: This position is responsible for assisting patients with their next appointment, scheduling out patients tests, and referring patients to other physicians.
LEADERSHIP COMPETENCIES

Takes responsibility for meeting general requirements, code of conduct, and policies/procedures as outlined in the Employee Handbook, as evidenced by the following which may be observed by physicians, management, staff, patients, visitors and co-workers: Maintaining attendance/punctuality in accordance with system policy & procedure Adhering to the dress code and maintaining a neat appearance Wearing and displaying ID badge appropriately at all times while on duty Maintaining an organized and clean work area Assisting with maintaining a neat and clean facility (pick up papers, reports, clean up spills, etc.) Maintaining confidentiality as outlined in practice policy & procedure Limiting equipment/computer use to work-related functions Willingly performs other duties as assigned Completing tasks within designated times lines as directed by management Following employee conduct expectations as outlined in the Employee Handbook Addressing co-workers in a courteous and professional manner at all times Communicating effectively and appropriately as may be observed by physicians, management, staff, patients, visitors and co-workers Demonstrating sensitivity to patients and their family members Maintaining self-control in times of organizational stress and/or patient crisis Recognizing unsafe and/or unusual situations and calling them to the attention of management
CUSTOMER SERVICE
Demonstrating an understanding of excellent customer service standards as evidenced by the following which may be observed by physicians, management, staff, patients, visitors and co-workers: Greeting everyone first Making a positive impression Treating others as guests Being respectful at all times to everyone Communicating clearly in a well-modulated tone and volume of speaking Serving others from a team-centered approach Projecting a positive image Making excellence the goal in everything you do DUTIES (examples) : Greet each patient as well as asking patient to step down to your window for assistance. Check patient out in Onco EMR. Complete all physician orders in patient chart. Enter in all appointments including placing radiology appointments on the schedule. Give patient all copies of appointments as well as any instructions and prep for outpatient test. Monitor and reschedule patients to an assigned physician schedule. Answer phone in your work area. Maintaining your work area, this position requires that you are at your desk at all times with the exceptions of duties requiring you to be away for short periods.
JOB QUALIFICATIONS
A high school degree or equivalent is preferred for this position. Computer skills are required. Minimum of 2-3 years experience in medical office is preferred. The individual must be able to communicate and understand verbal and written English language.
PHYSICAL DEMANDS & WORKING CONDITIONS OF JOB
This position requires sitting, walking, some bending, stooping, and stretching to include lifting up to 25 pounds. There is the potential for exposure to communicable illness from contagious patients. There is extensive computer work, including viewing computer screens for long periods of time.
