Job Description

Basic Functions and Scope of Responsibility The Community Health Director leads the YMCA's efforts to improve community health outcomes by developing, implementing, evaluating, and growing evidence-informed chronic disease prevention and management initiatives while overseeing the Y's wellness programs, staff, and facilities. This position builds partnerships with hospitals, health systems, medical providers, public-health agencies, payers, employers, and community organizations to expand access to integrated health and wellness services. The Director also advances secure referral, data-exchange, and outcomes-reporting processes through HISP, health information exchange, electronic referral, and related software solutions to support coordinated programming with medical partners. In addition, the Director supervises the Wellness Coordinator and provides leadership for group exercise, personal training, staff development, and member experience, with accountability for program quality, equitable access, measurable impact, financial stewardship, and sustainable growth consistent with the YMCA's mission. Principal Responsibilities
    Community Health Strategy & Program Growth:
    Assess community health needs and service gaps using local data, participant feedback, partner input, and YMCA priorities. Develop, implement, evaluate, and grow evidence-informed chronic disease programs, including diabetes prevention, blood-pressure control, healthy weight, cancer survivorship, mobility, and falls prevention. Establish clear referral, enrollment, delivery, safety, and follow-up processes while building sustainable funding through grants, contracts, fees, reimbursement opportunities, and partner support. Ensure programs are accessible, culturally responsive, and inclusive of individuals facing financial, transportation, language, disability, or other barriers.
      Healthcare Partnerships & Integrated Delivery:
      Build partnerships with healthcare providers, public-health agencies, payers, and other referral organizations to develop integrated community health programs. Establish clear roles, referral criteria, communication, consent, clinical escalation, and shared outcome measures, including secure closed-loop referral and reporting processes. Represent the YMCA in community health and population-health initiatives while ensuring clinical decisions and services remain within the scope of appropriately licensed professionals.
        Health Technology, Data, & Evaluation:
        Lead technology solutions that support secure referrals, enrollment, care coordination, outcomes tracking, and partner reporting, including connections with health information exchange programs. Ensure appropriate privacy, security, agreements, access controls, and staff training. Develop dashboards and reports to evaluate participation, access, outcomes, partner satisfaction, and financial performance, using data to improve programs, guide resources, meet funding requirements, and communicate community impact.
          Staff Leadership & Development:
          Lead wellness operations through the Wellness Coordinator, ensuring effective scheduling, program quality, documentation, and member support. Oversee group exercise, personal training, wellness orientations, health coaching, and specialty programs; recruit, develop, and retain qualified instructors and trainers. Maintain credentialing, safety, continuing education, emergency readiness, and service standards. Support fitness equipment planning and forecasting. Obtain instructor-trainer certification to teach YMCA staff CPR, First Aid, and AED.
            Financial, Marketing, & Organizational Leadership:
            Partner with Marketing to promote programs, educate providers, recruit participants, and communicate community impact. Pursue grants, contracts, donations, and other funding opportunities while supporting related budgets and reporting. Lead or support mission-aligned health, wellness, and community events, serve as Manager on Duty, participate in the Annual Campaign, and contribute to association-wide planning and leadership.
              Other Duties:
              Assigned as necessary by the Membership & Operations Director. Training, Skills, Knowledge and/or Experience Required Education and Experience
              • Bachelor's degree in public health, community health, health promotion, nursing, healthcare administration, population health, exercise science, social work, or a closely related field; an equivalent combination of relevant education and experience may be considered.
              • At least two (2) years of experience in community health, chronic disease prevention or management, population health, clinical-community partnerships, healthcare operations, wellness leadership, or a related field.
              • At least two (2) years of staff supervision, program leadership, or cross-functional team leadership.
              • Demonstrated experience developing, evaluating, and improving health programs; using data to demonstrate results; and building effective partnerships with clinical, community, and organizational stakeholders.
              • Strong knowledge of privacy and secure health-information practices, along with budgeting, project management, communication, organizational, and technology skills; experience with referral, care-coordination, membership, or outcomes platforms is preferred.
              • First Aid, CPR, and AED certification obtained within 30 days of employment and maintained throughout employment. In addition, Instructor-Trainer certification in CPR, First Aid, and AED. Preferred Education, Credentials, and Background
              • Certified Health Education Specialist (CHES).
              • Registered Nurse (RN) or other relevant licensed health-professional background, particularly with care management, community health, population health, nursing leadership, or chronic disease experience.
              • Relevant clinical exercise, health coaching, or chronic disease credentials, such as ACSM Certified Clinical Exercise Physiologist or Exercise Physiologist, NBC-HWC, ACE Health Coach, CDC-recognized National Diabetes Prevention Program Lifestyle Coach training, or comparable YMCA credentials.
              • Grant development, contract management, payer relationships, reimbursement strategy, or value-based care collaboration.

              • Must have high level of alertness, concentration, initiative, and ability to work independently.
              • Ability to lift 50 lbs. and stand, walk, or sit for extended periods of time; occasionally stoop/bend.

              Pay:
              $55,000.00 - $66,000.00 per year
