LifeSpring is seeking a dedicated and compassionate Outpatient Case Manager Coordinator to lead the development and implementation of case management services and to implement a structured curriculum for adults living with severe mental illness who utilize our Community Resource Center. This role is central to the success of our Adult Day Service Program, which provides a supportive, recovery-oriented environment for participants to engage in skill-building, social connection, and personal growth. The ideal candidate will combine strong leadership and organizational skills with a deep understanding of mental health recovery. Responsibilities include curriculum development, event planning, scheduling, grant writing, and oversight of daily program operations and CRC staff.
Key Responsibilities:
Design and implement a trauma-informed, person-centered curriculum tailored to the needs of adults with severe mental illness Coordinate and supervise outpatient case management team and lead daily CRC programming, including life skills, wellness activities, creative expression, and community engagement Plan and manage community-based events and enrichment opportunities that support social inclusion and recovery Develop schedules for staff and volunteers, ensuring consistent and high-quality program delivery Write and contribute to grants and funding proposals to support program sustainability and growth Collaborate with clinical staff, peer support specialists, and community partners to ensure services are holistic and integrated Monitor and evaluate program outcomes, making data-informed improvements Foster a safe, respectful, and empowering environment for all participants
Qualifications:
Bachelor's degree in Social Work, Psychology, Public Health, Human Services, or related field Minimum 2 years' experience in program coordination, community mental health, or related setting Demonstrated experience in curriculum development, group facilitation, and event planning Strong grant writing and organizational skills Understanding of trauma-informed care, and person-centered planning Excellent interpersonal, communication, and leadership abilities
Preferred Skills:
Experience working with adults with SMI (Severe Mental Illness).
