We are seeking a clinically sharp, organized, and empathetic Care Manager Nurse to lead the clinical care and discharge planning for up to 25 rehab residents. In this vital role, you will bridge the gap between treatment, recovery, and a successful return home.

Key Responsibilities Care Coordination:

Oversee the comprehensive clinical care plans for all short-term rehab residents, updating goals dynamically based on patient progress.

Interdisciplinary Collaboration:

Act as the central hub connecting physical/occupational therapy teams, physicians, families, and floor nurses to ensure seamless care execution.

Discharge Planning:

Lead the safe discharge process, coordinating home health services, ordering medical equipment, and educating families on post-facility care.

Utilization Review:

Review patient charts and progress notes to ensure compliance with Medicare, managed care, and private insurance clinical documentation guidelines.

Family Liaison:

Serve as the primary point of contact for resident families, providing regular updates on clinical milestones, therapy timelines, and next steps.

Clinical Supervision:

Provide oversight and support to floor nurses and nursing assistants on the rehab wing, ensuring clinical benchmarks are met.

Job Requirements Licensure:

Active, unencumbered Registered Nurse (RN) or Licensed Practical Nurse (LPN) license in the state of Louisiana.

Experience:

Minimum of 1 year of nursing experience, preferably in a skilled nursing facility (SNF), sub-acute rehab setting, or acute care hospital.

Soft Skills:

Exceptional organizational skills, the ability to multi-task in a fast-paced setting, and top-tier communication skills for handling sensitive family

Pay:

From $26.00 per hour Expected hours: 40.0 per week
