Job Description
I. Position Summary The Clinical Manager is responsible for the daily supervision, coordination, and evaluation of all clinical operations to ensure safe, effective, and compliant delivery of home health services. This position provides leadership to the interdisciplinary team, oversees adherence to the Plan of Care, and ensures that all services meet Medicare Conditions of Participation, MassHealth, and CHAP regulatory requirements. II. Essential Duties and Responsibilities A. Leadership & Oversight Provides daily oversight of all clinical staff and patient care activities. Conducts morning huddles and case conferences to coordinate admissions, recertifications, and discharges. Reviews and approves OASIS assessments, 485 Plans of Care, orders, and documentation within agency timelines. Ensures compliance with physician orders, visit frequencies, and plan of care goals. Acts as a liaison between field clinicians, physicians, and office administration. B. Staff Management Supervises, trains, and evaluates field clinicians (RNs, LPNs, Therapists, Aides). Monitors productivity, timeliness of documentation, and clinical accuracy. Conducts joint home visits and competency evaluations annually and as needed. Oversees onboarding, orientation, and mentorship of new clinicians. Promotes a positive, collaborative culture focused on patient outcomes and regulatory compliance. C. Clinical Quality & Compliance Reviews clinical documentation for completeness, accuracy, and regulatory compliance. Ensures timely submission of OASIS-E, SOC, ROC, Recert, and Discharge documentation. Monitors missed visits, incidents, and infection control logs; ensures corrective action is taken. Participates in the agency's QAPI (Quality Assurance and Performance Improvement) program. Ensures compliance with
