Job Description

LifeBridge Health, Inc. seeks a Nurse Navigator to guide patients and families through complex care pathways across our regional health system. The Nurse Navigator coordinates appointments, diagnostics, referrals, and transitions of care, serving as a primary point of contact and advocate. This role provides education, symptom management guidance, and emotional support, collaborating closely with physicians, social workers, and allied health teams. The Nurse Navigator tracks outcomes, removes barriers to care, supports quality and safety initiatives, and promotes culturally competent, patient centered care in alignment with our community-focused mission. Responsibilities Serve as primary point of contact and advocate for assigned patients and families Coordinate appointments, diagnostics, referrals, and transitions of care across settings Provide patient and family education on diagnoses, treatments, and self-management Identify and address barriers to care, including social determinants and access issues Collaborate with physicians, nurses, social workers, and allied health professionals Monitor patient progress and outcomes, escalating clinical concerns appropriately Support symptom management and triage non-emergent patient concerns Document all navigation activities in the electronic health record accurately and timely Participate in quality improvement, population health, and care pathway initiatives Promote culturally competent, patient-centered, and community-focused care Required Skills Clinical nursing assessment Care coordination and discharge planning Patient and family education Electronic health record (EHR) documentation Interdisciplinary collaboration Case management and utilization management Symptom management and triage Population health and outcomes tracking Cultural competence in patient care Motivational interviewing and counseling
