Job Description
Hegira Health, Inc. (HHI), a private non-profit corporation, is one of the largest freestanding behavioral healthcare agencies in the area. HHI, accredited by the Joint Commission and licensed by the State of Michigan, provides a broad array of mental health and substance abuse treatment and prevention services to individuals of all ages. The Assertive Community Treatment and Community Programs Coordinator oversees the ACT and community programs with locations in Westland and Lincoln Park.
REQUIREMENTS
A Master's Degree from an accredited college or university, specializing in psychology, social work, counseling, or other related human services field Current, appropriate licensure/certification issued by the State of Michigan LMSW, LPC or LLP (FULL LICENSE) Knowledge and experience of/with ACT Five years of progressively more responsible experience in a related mental health and substance abuse services field One year of supervisory experience Experience with fiscal management of clinical service operations and understand various types of payor reimbursement methodologies.RESPONSIBILITIES
(summary): Maintain up-to-date knowledge of treatment protocols appropriate to services supervised. Assign staff assignments based on based on skill assessment (e.g., population, training, credentials, etc.). Assign and notify staff of activity expectations consistent with goals. Monitor staff activity to established annual goals and objectives, quality measures, and policy and procedure and regularly report to Director of Adult Services. Community Liaison Serve as back-up program liaison to payor sources, community outreach organizations, etc., in the absence of the Director of Outpatient Adult Services . Supervision Routinely review staff performance. Provide team and individual staff supervision. Indicate review in clinical records by signatures on record review documents. Ensure staff practice is maintained within the limits of job requirements and established areas of competency.Ensures completion of the following:
- Comprehensive Case Management Assessment and/or Integrated Biopsychosocial Assessment
- Person-Centered Planning and Individual Plan of Service
- Coordination of Care with other services/providers is completed and maintained.
- Direct service hours, per policy.
