A patient is leaving the hospital. They need nursing at home. Or physical therapy. Or help managing a new diagnosis. The referral comes in. Now someone has to make it happen. At Home Health Care, Inc., our Intake team helps move patients from referral to admission and ultimately to care in their own homes . That takes more than data entry. It takes someone who can review the information, recognize what's missing, communicate with the right people, follow up, and keep the process moving. That's where you come in. Why This Role Matters When intake works well, patients receive care faster. Referral sources know what's happening. Clinicians receive the information they need. Families know what to expect. And fewer details fall through the cracks. You'll become one of the people responsible for making that happen. What We Offer $24-$28/hour. Full-Time position. Competitive benefits package. Locally owned Minnesota organization with more than 30 years of experience. Your Job Is to Keep the Referral Moving A referral may arrive without everything we need. Maybe an order is missing. Maybe documentation needs clarification. Maybe insurance needs to be verified. Maybe the clinical information isn't complete. You won't simply put it aside. You'll figure out what's needed next. You'll communicate with referral sources, obtain documentation, verify information, update the EMR, and coordinate internally so qualified patients can move toward admission. What You'll Do Review and process incoming home health referrals. Screen referrals for admission requirements. Verify payer, insurance, and reimbursement eligibility . Review orders, diagnoses, clinical documentation, and insurance information. Contact referral sources for missing documentation. Conduct intake conversations with clients and families. Enter accurate patient information into the EMR. Communicate with hospitals, clinics, physicians, facilities, and other referral partners. Coordinate with internal clinical and operational teams. Track referral status and follow up on outstanding items. Help create a responsive and organized admission experience. HUC Experience? You May Already Have the Skills We Need. If you've worked as a Health Unit Coordinator, think about what you've already learned. Prioritization. Medical terminology. Clinical communication. Documentation. Technology. Follow-through. Keeping calm when five things need attention at once. Those skills can translate extremely well to home health intake. We're especially interested in candidates with experience as a: Health Unit Coordinator (HUC). Unit Coordinator or Health Unit Clerk. Intake Coordinator or Intake Specialist. Admissions Coordinator. Referral Coordinator. Patient Care Coordinator. Medical Office Coordinator. Patient Administration Specialist. Hospital or Clinic Administrative Coordinator. Home Health Intake Specialist. Home health experience is strongly preferred, but candidates with strong healthcare intake, admissions, referral, or unit coordination experience are encouraged to apply. What We're Looking For Healthcare office or administrative experience. Intake, admissions, referral, or HUC experience strongly preferred. Familiarity with medical and insurance terminology. Strong EMR and computer skills. Excellent attention to detail. Ability to manage multiple referrals and priorities simultaneously. Professional communication with clinical teams and referral sources. Strong follow-through. Ability to recognize missing information and take action to obtain it. Collaborative, solutions-oriented approach. Be the Person Who Gets Care Started.
