Behavioral Health Healthcare Home Schedule:
M-F 8-5 Are you passionate about bringing physical and behavioral health care together to support people on their wellness journey? As a Nurse Care Manager in our Behavioral Health Healthcare Home program, you'll be a key part of a multidisciplinary team providing holistic care to clients with serious mental illness and co-occurring conditions. You'll deliver targeted interventions that help close health gaps and empower individuals to lead healthier, more independent lives. This is a great opportunity for someone who thrives in a collaborative, community-based setting and believes in the power of integrated care to improve long-term outcomes for individuals with behavioral health challenges.
