Job Description
Americare Senior Living seeks a Health and Wellness Director for our Memory Care community. In this on-site leadership role, you will oversee resident health services, develop and monitor individualized care plans, and ensure high-quality, person-centered dementia care. You will supervise and coach caregiving and clinical staff, coordinate with physicians and families, manage medications, and support regulatory compliance and safety. This position is ideal for a nurse or experienced clinical leader passionate about improving quality of life for seniors in a collaborative, mission-driven environment. Responsibilities Lead and manage health and wellness services within the memory care community Develop, implement, and update individualized care plans for residents Supervise, schedule, and mentor caregiving and clinical staff Oversee medication management and clinical documentation Ensure compliance with state regulations, policies, and safety standards Collaborate with physicians, therapists, and external providers Educate and support residents' families on dementia care and progression Monitor resident health status and coordinate interventions as needed Lead quality improvement initiatives and incident follow-up Support a resident-centered, compassionate culture aligned with Americare's mission Required Skills Memory care program management Care planning and assessment Staff supervision and scheduling Geriatric and dementia care knowledge Regulatory and compliance knowledge Medication management oversight Family and resident communication Incident reporting and quality improvement Electronic health record (EHR) documentation Interdisciplinary team coordination
