We are currently seeking a compassionate and highly skilled Hospice Clinical Manager to oversee our hospice care team. This leadership role involves ensuring the delivery of exceptional end-of-life care, maintaining adherence to the highest standards of patient care, and supporting the emotional needs of our patients and their families. The ideal candidate will embody our commitment to patient-centered care and have a passion for enhancing the wellbeing of our clients within the hospice setting. Responsibilities Lead and manage a multi-disciplinary team including nurses, therapists, and social workers to ensure quality of care. Develop and implement patient care plans and assess ongoing patient and family satisfaction. Ensure compliance with all federal, state, and local regulations and adherence to hospice policies and procedures. Conduct regular staff meetings and training sessions to ensure team is up-to-date with the latest in palliative care practices. Manage budgeting and resource allocation for the hospice program. Serve as a key point of contact for family members, providing timely updates and addressing concerns. Review and analyze patient outcomes facilitating continuous improvement in care delivery. Required Skills Strong leadership and team management skills In-depth knowledge of hospice care and palliative treatment options Exceptional interpersonal and communication skills Ability to handle high-stress and emotional situations Proficiency in EMR (Electronic Medical Records) and MS Office
