This leadership role involves ensuring the delivery of exceptional end-of-life care, maintaining adherence to the highest standards of patient care, and supporting the emotional needs of our patients and their families. Responsibilities Lead and manage a multi-disciplinary team including nurses, therapists, and social workers to ensure quality of care. Develop and implement patient care plans and assess ongoing patient and family satisfaction. Ensure compliance with all federal, state, and local regulations and adherence to hospice policies and procedures. Conduct regular staff meetings and training sessions to ensure team is up-to-date with the latest in palliative care practices. Manage budgeting and resource allocation for the hospice program. Serve as a key point of contact for family members, providing timely updates and addressing concerns. Review and analyze patient outcomes facilitating continuous improvement in care delivery.
