Resident Care Coordinator (RCC) Mill Creek Manor - Statesville, NC Full-Time Job Description- Mill Creek Manor is seeking an experienced, compassionate, and highly organized Resident Care Coordinator (RCC) to join our leadership team. The Resident Care Coordinator plays a key role in overseeing the day-to-day delivery of resident care and ensuring that our residents receive safe, compassionate, and consistent services. This position works closely with the Executive Director, care staff, residents, families, healthcare providers, pharmacy, and other members of the facility team. We are looking for someone who is a strong leader, communicates effectively, holds staff accountable, and genuinely cares about the quality of life and well-being of our residents. Responsibilities Coordinate and oversee the daily care needs of residents in accordance with individual care plans and physician/provider orders. Supervise, support, and provide direction to Med Techs, PCAs, and other direct-care staff . Help ensure appropriate staffing and coverage within the resident-care department. Monitor resident conditions and promptly communicate changes or concerns to the appropriate healthcare provider, family, and facility leadership. Ensure resident assessments, care plans, medication records, and other required documentation are accurate, current, and properly maintained. Coordinate resident appointments, provider visits, pharmacy needs, medication-related follow-up, and other healthcare services. Communicate professionally and compassionately with residents and their families regarding care needs and concerns. Participate in resident admissions, transitions, and ongoing care coordination. Assist with orientation, training, coaching, and performance management of direct-care staff. Monitor staff performance and ensure assigned care and documentation are completed appropriately. Help maintain compliance with North Carolina assisted-living regulations, facility policies, infection-control practices, and resident-rights requirements. Participate in quality improvement efforts and help identify and correct resident-care or documentation deficiencies. Respond promptly to resident incidents, changes in condition, family concerns, and other care-related issues. Participate in leadership and morning meetings and communicate important resident-care updates to facility leadership. Work collaboratively with the Executive Director and other department leaders to maintain a safe, respectful, and resident-centered environment. Qualifications Previous experience in assisted living, senior living, long-term care, memory care, or a similar healthcare environment strongly preferred. Previous experience supervising or leading PCAs, Med Techs, CNAs, or other direct-care staff preferred. Strong understanding of resident care, medication management processes, documentation, and regulatory compliance. Excellent leadership, communication, organization, and problem-solving skills. Ability to effectively communicate with residents, families, employees, healthcare providers, and facility leadership. Strong attention to detail and ability to follow through on multiple priorities. Comfortable using electronic healthcare/documentation systems and learning new technology. Must maintain resident confidentiality and comply with HIPAA and facility policies.
