Patient Care Coordinator, Interventional Radiology Vantage Vascular Fair Lawn, NJ Full-Time | In-Person Monday-Friday, 9:00 AM-5:00 PM $24.00-$26.00 per hour Job Summary Vantage Vascular is seeking a highly organized, dependable, and patient-focused Patient Care Coordinator to join our outpatient Interventional Radiology practice in Fair Lawn, NJ. This is a clinical support and patient coordination role comparable in level to a Medical Assistant position . The Patient Care Coordinator works closely with patients primarily by phone, helping guide them through scheduling, procedure preparation, required testing, medical records collection, and follow-up. This person will serve as an important point of contact for patients throughout the procedural process and will work closely with our clinical team to ensure patients are prepared and ready for their procedures. A significant part of this position involves speaking with patients on the phone and making outbound calls to schedule consultations, procedures, and follow-up appointments. The ideal candidate should be comfortable communicating with patients throughout the day, following up on outstanding items, and taking ownership of the scheduling and coordination process. The position also includes obtaining outside medical records, coordinating pre-procedure requirements, uploading documentation into the electronic medical record, answering incoming phone calls, faxing documents, and supporting the clinical workflow of the practice. Responsibilities Make outbound phone calls to patients to schedule procedures, consultations, and follow-up appointments Proactively follow up with patients who have not yet scheduled their procedure or appointment Answer incoming patient phone calls and assist with scheduling, questions, and coordination of care Schedule and coordinate outpatient Interventional Radiology procedures Communicate procedure dates, times, locations, preparation requirements, and other important information to patients Provide patients with pre-procedure instructions Coordinate required pre-procedure testing and documentation, including: Laboratory testing Imaging Medical clearance Medication instructions Other required records or testing Follow up with patients to ensure all required pre-procedure items are completed prior to their procedure Obtain outside medical records, imaging reports, laboratory results, physician notes, and other documentation Contact physician offices, hospitals, imaging centers, and other healthcare facilities to request patient records Upload, scan, and organize documents within the electronic medical record Maintain accurate and up-to-date patient charts Fax medical records, orders, requests, and other clinical documentation Assist with scheduling imaging, laboratory appointments, consultations, procedures, and follow-up visits Communicate with physicians, clinical staff, procedural facilities, and team members regarding patient care Track outstanding patient tasks and ensure required documentation is received Assist with procedure rescheduling, cancellations, and other scheduling changes Support the clinical team with patient coordination needs as required What We Are Looking For We are looking for someone who is proactive, organized, personable, and comfortable taking ownership of patient follow-up . This is not simply a front-desk or scheduling position. The Patient Care Coordinator plays an important role in making sure patients successfully move through the process from consultation to procedure.
The ideal candidate is comfortable:
Spending a significant portion of the day speaking with patients by phone Making outbound calls to schedule patients Following up with patients multiple times when necessary Managing multiple patients and outstanding tasks simultaneously Obtaining missing medical records and documentation Communicating clearly and professionally with patients Working independently and following tasks through to completion Maintaining accurate documentation in the EMR Qualifications Previous experience in a physician office, medical practice, outpatient center, hospital, or other healthcare setting preferred Experience with patient scheduling, procedure scheduling, patient coordination, or Medical Assistant work strongly preferred Medical Assistant experience or certification is a plus, but not required Comfortable making frequent outbound patient phone calls Strong telephone and patient communication skills Experience working with electronic medical records eClinicalWorks (eCW) experience preferred Knowledge of basic medical terminology preferred Strong organizational skills and attention to detail Ability to multitask and manage multiple patient needs simultaneously Strong computer skills Professional, dependable, and punctual Excellent verbal and written communication skills Strong follow-through and ability to work independently Experience in Interventional Radiology, vascular surgery, cardiology, urology, orthopedics, surgery, pain management, or another procedure-based medical specialty is a plus, but not required. Compensation $24.00-$26.00 per hour , depending on experience. Our team works closely with patients throughout the entire treatment process, and the Patient Care Coordinator is an important part of ensuring that each patient receives timely communication, appropriate scheduling, and a smooth experience from consultation through procedure and follow-up.
Pay:
$24.00 - $26.00 per hour Expected hours: 40.0 per week
