Care Manager Pay:
From $53.00, based on license and experience Job type: Part-time, W-2, 15 to 30 hours per week The Role We're looking for a Care Manager to join our growing team and walk alongside clients and families as needs change over time. You'll assess situations, develop clear care plans, and coordinate medical, social, and community supports, often during stressful or uncertain moments. You will be paid for all client related phone calls, emails, documentation, drive time, client visit, care coordination, appointment attendance, and paid training. Location This is a field role where you will be driving to clients' homes, hospitals, and facilities across Hudson County most days you work, so you need to already live in Hudson County or within about 15 minutes of its border. We assign clients by geography on purpose, so our care managers spend their time with families instead of on the road. The work Families come to us when things feel overwhelming: a health decline, a fall, a diagnosis, or too many decisions landing at once. You are the person who walks in, sorts out what is actually happening, and builds a plan the family can live with. Day to day, that means comprehensive assessments (medical, functional, psychosocial), individualized care plans, coordinating across doctors, facilities, and community services, attending key medical appointments, supporting medication oversight and refills, and keeping families and responsible parties clearly informed. Your first weeks You will not be handed a caseload cold. You will shadow experienced care managers on real client visits before taking your own, so you start with context and a team behind you. Training is paid, both upfront and ongoing. Your week Monday through Friday, daytime. You build your own visit calendar around the clients assigned to you. Occasional after-hours or weekend calls come up when a client has an emergency, and those are paid at an after hours rate. About Peace Care Experts We are a private-pay care management practice in Chester, New Jersey, with more than twenty years of work behind us and 20+ credentialed professionals on the team: nurses, social workers, occupational therapists, and recreational therapists. We serve older adults, adults with mental illness, and adults with neurological and developmental conditions, along with their families. What you bring Active New Jersey license: RN, LPN, PT, OT, or SW (MSW, LSW, LCSW) Valid driver's license and daily access to a car Calm, compassionate presence and strong clinical judgment Clear communication and follow-through Comfort working independently in the field Care management or geriatrics experience preferred CCM or CMC certification a plus Pay and benefits $53.00 per hour, based on license and experience Paid training, travel time, and mileage reimbursement Paid time off and sick pay 401(k) with employer match Company laptop and cell phone Target start date: October 4, 2026.
