Know Your Team:
Hackensack Meridian Pascack Valley Medical Center is a 128-bed, full-service, acute-care community hospital with a new emergency department, a state-of-the-art maternity center, a women's imaging center, and an ICU. Responsibilities The M anager, Case Management is responsible for assisting the case management director with hospital case management, social work , transition management, and operational management of the case management d epartment .
Qualifications Job Requirements :
RN, Associate's degree in nursing or higher. Social Work degree, BSW or higher. 1+ years' experience in a case management leadership role.
Preferred Job Requirements:
2+ years' experience in acute hospital-based case management. Certification in case management (ACM or CCM). Rate of pay is determined based on experience and education and may include other pay components such as differentials and call pay based on role.
