Job Description

As a Foster Care Community Service Provider, you will provide home and community-based services to help children with emotional and mental health challenges. You will travel locally to work with children and families in their homes and communities, providing services to address identified client needs. You will report to the clinical supervisor. You will work onsite and in clients' homes and the community.
Responsibilities:
Provide services within a family-driven care network using a client centered strength focus lens. Work with family members to identify goals for treatment Provide services as identified in the treatment plan Coordinate with other services providers to ensure consistency of treatment Coordinate with the caregiver on scheduling and provision Participate in any relevant treatment or service plan meetings Complete all necessary documentation.
Requirements and Education:
Bachelor's degree and two years of experience in related field required.
