  • 75,000 per year RN or Social Worker Preferred We are seeking a strong, hands-on Program Director to lead the reopening and growth of our Adult Day Health Care program in Greenport, NY.
This is a great opportunity for someone who wants to take ownership of a program, build something successful, and create a welcoming, active environment that participants genuinely look forward to attending. The Program Director will oversee the day-to-day operation of the program and work closely with nursing, social work, activities, therapy, dietary, transportation, marketing, and facility leadership. A major part of this role will also be building census, developing referral relationships, and making sure the program is organized, compliant, and running smoothly. Responsibilities Lead the reopening, daily operation, and continued growth of the Adult Day Health Care program Create a welcoming, engaging, and well-run program for participants Oversee admissions, referrals, assessments, care planning, documentation, and ongoing participant needs Coordinate with nursing, social work, therapy, dietary, activities, transportation, and other departments Build census through strong relationships with referral sources, hospitals, MLTCs, community organizations, physicians, senior housing, and local providers Follow up on prospective participants and help move referrals through the admission process Oversee staff and maintain accountability across the program Ensure compliance with all applicable Department of Health requirements and program policies Prepare for surveys and maintain organized, survey-ready records Monitor attendance, transportation, staffing, and overall program performance Work closely with leadership to identify opportunities to grow and improve the program Bring new ideas to activities, programming, outreach, and participant engagement Qualifications Must have experience in adult day health care, long-term care, home care, case management, geriatrics, behavioral health, senior services, or another related healthcare setting RN or social worker strongly preferred Previous management or program leadership experience strongly preferred Strong organizational and follow-up skills Comfortable working with clinical documentation, care plans, referrals, and admissions Strong communication and relationship-building skills Able to take ownership, solve problems, and keep multiple areas moving at once Experience with marketing, community outreach, referral development, or census growth is a plus We are open to candidates from different healthcare backgrounds. The right person does not necessarily need to have previously held the exact title of Adult Day Health Care Program Director.
