Job Description

Job description We are currently looking for a Case Manager (Director of Case Management) to join our amazing team at the beautiful and luxurious Bristal at Lake Success. The Case Manager will act as a liaison between Residents, their families, administration staff and community-based service providers.
Additional responsibilities include:
Acting as support to Residents, providing orientation, social/emotional support and arranging for needed services Help oversee the Wellness Department, including a dedicated Alzheimer's/Dementia unit Assess prospective Residents physical, mental and emotional needs and current status Oversee the admissions and discharge process; maintaining accurate case notes and assists Residents in further placement upon discharge Must develop and keep accurate client records, logging Resident interactions and/or impressions and maintaining requisite case notes on Residents Responsible for coordinating ISP process, Incident Reporting, Falls Management Program as well as coordinating outside services such as Rehab, Homecare, Psychological Services. Requirements A current NYS RN or LPN license or LCSW, LMSW, Social Work background Experience working in an assisted living, skilled nursing facility or long-term care environment A successful background in nursing, case work, social work or health advocacy role desirable Minimum of four(4) years' experience with an adult/geriatric population working in an admissions, assessment, family liaison or discharge planning capacity preferred Ability to understand the needs of a senior and dementia-based population Top notch communication skills, both verbal and written Strong attention to detail, exceptional follow through and time management skills Highly organized, ability to multi-task and thrive in and extremely fast paced, ever changing environment Exceptional interpersonal and relationship building skills Ability to converse well with a diverse audience in a clear and concise manner Demonstrated leadership skills Schedule to include one weekend day per week for a Sunday-Thursday day shift schedule 9am-5:30pm

Pay:
$85,000.00 - $95,000.00 per year
