Role:
The HEAL Project Coordinator leads the implementation of SEPA Mujer's Cultural Bridges for Health Equity / Puentes Culturales Para la Equidad de Salud project. This initiative empowers underserved Latina immigrant women and low-income families in Suffolk County to overcome systemic healthcare barriers, navigate health systems with confidence, and build community leadership. The Coordinator oversees an 11-month Health Equity Advocacy Leadership (HEAL) Cohort, instructs interactive classes, delivers health-cultural competency training to major healthcare provider networks, executes the annual Health Equity Summit, and addresses community fears surrounding healthcare access, language rights, and privacy concerns.
Organize, recruit, retain, and manage a minimum of 50 community members for an 11-month HEAL Leadership Cohort through grassroots outreach, social media campaigns, chapter meetings, and partner referrals.
Class Facilitation:
Lead and instruct engaging workshops and classes covering health literacy, patient rights, digital literacy in healthcare, culturally tailored health education, accessing language services, and social determinants of health.
Graduation & Leadership:
Host a formal graduation ceremony awarding Certificates of Completion and transitioning graduates into ongoing community health advocacy roles. 2.
Healthcare Systems Navigation & Client Transportation Health Systems Navigation:
Guide community members through healthcare systems, assist with scheduling primary care appointments, and establish connections with specialty care providers.
Insurance & Public Benefits Guidance:
Assist clients in understanding private medical insurance policies, navigating coverage options, and applying for public benefits and programs available to uninsured individuals.
Direct Client Transportation:
Transport clients directly to medical appointments across the Long Island service area as needed using personal vehicles (reimbursed).
Site Outreach & Tabling:
Conduct regular tabling, one-on-one education, and outreach at healthcare partner sites.
Resource Distribution:
Collaborate with the Bilingual Community Liaison and the Equity & Community Empowerment (ECE) Department to distribute 1,000+ bilingual resource packages, provider directories, and "Know Your Rights" guides. 3.
Health Provider Cultural Competency & Systems Advocacy Provider and Partner Training:
Facilitate Health-Cultural Competency and Trauma-Informed Care workshops for healthcare professionals and leadership across 11 partner institutions.
Coalition & Partner Collaboration:
Work closely with regional health partners, advocacy coalitions, and task forces to address community fears when seeking medical care.
Health Equity Summit:
Coordinate and execute the annual Health Equity Summit to train healthcare and human service professionals on culturally sensitive service delivery. 4.
Data Collection, Metrics & Grant Reporting Outcome Tracking:
Monitor program progress to hit established grant deliverables: Minimum 50 HEAL Cohort participants graduating with certificates. Minimum 11 Community Health Partners trained in cultural competency. Minimum 500 total community members directly reached and supported.
Target minimum navigation metrics:
135 primary care appointments scheduled, 75 attended visits, 50 specialist connections, 50 health insurance utilization support, 85 public benefits enrollments, and 105 patient portal setups.
Evaluation:
Administer pre- and post-workshop evaluation surveys to evaluate class effectiveness, refine curricula, and prepare reports for leadership and funders.
Qualifications and Skills:
Education:
Bachelor's degree in Public Health, Social Work, Community Health, Human Services, or a related field required.
Experience:
2-3 years of experience in community organizing, public health outreach, project coordination, supervision of staff/volunteers, and case management.
Instructional Experience:
Demonstrated experience teaching, instructing classes, or facilitating workshops for adult learners and community members. Community engagement and recruitment for the entire project.
Health System & Insurance Knowledge:
Strong working knowledge of local healthcare systems, clinical workflows, and patient navigation strategies. Solid understanding of medical insurance coverage (Medicaid, Medicare, private insurance). Demonstrated knowledge of public benefits, Safety Net options, and financial assistance programs for uninsured or underinsured immigrants.
Partner & Cohort Management:
Proven ability to organize structured cohorts and build productive collaborative relationships with institutional healthcare partners.
Language Proficiency:
Must be professionally bilingual in Spanish and English with excellent verbal, written, and translation skills.
Transportation Requirement:
Must possess a valid driver's license, reliable personal vehicle, clean driving record, and full willingness to transport clients to medical appointments and other needs across Long Island.
Technical Skills:
Proficiency in Google Workspace, Canva, and data tracking tools.
