Serene Home Nursing Agency is looking for a friendly, organized, and motivated Licensed Practical Nurse (LPN) to join our team as a Patient Care Coordinator. If you enjoy working with patients, families, nurses, and other healthcare professionals—but are interested in moving beyond traditional bedside nursing—this position offers an opportunity to use your clinical knowledge in a new and important role. As a Patient Care Coordinator, you'll help keep the many moving pieces of home healthcare working together. You'll communicate with patients and families, support our field nurses, help solve day-to-day patient care and staffing concerns, and make sure important issues are recognized and addressed. This is a great fit for an LPN who is personable, dependable, comfortable making decisions, and good at figuring things out when the day doesn't go exactly according to plan. What You'll Do As part of our Patient Care Coordination team, you will:

• Communicate regularly with patients, families, nurses, and other members of the healthcare team
• Help coordinate patient services and maintain reliable nursing coverage
• Respond to day-to-day patient care and staffing concerns
• Recognize urgent or time-sensitive situations and help determine the appropriate next steps
• Work with field nurses to address scheduling, patient care, and case-related needs
• Help resolve call-outs, schedule changes, and other unexpected coverage issues
• Maintain accurate information and documentation within HHAeXchange
• Work closely with other office departments to keep communication and patient services running smoothly
• Assist with field-based responsibilities when needed, including supporting patients, families, and field staff
• Follow agency policies, procedures, and applicable healthcare requirements
• Participate in the agency's rotating on-call responsibilities for patient care and staffing matters outside of regular office hours No two days in home healthcare are exactly the same.

Training will be provided. Schedule Monday-Friday 8:00 AM-5:00 PM 1-hour unpaid lunch: 1:00 PM-2:00 PM This is a primarily office-based position with hybrid and field-based responsibilities as needed to support patients, families, field staff, and agency operations. On-Call Responsibility Patient care doesn't always fit neatly into regular business hours. The Patient Care Coordinator participates in the agency's rotating on-call schedule and may be responsible for responding to time-sensitive patient, family, or staffing matters outside of normal office hours. On-call responsibilities may include communicating with patients and field staff, responding to call-outs or changes in coverage, helping resolve urgent staffing concerns, and coordinating an appropriate response when an issue cannot wait until the next business day.
