You'll conduct client assessments, develop individualized care plans, coach caregivers, and help ensure every client receives the highest level of service. This role offers a balance of non-medical oversight, relationship building, leadership, and community engagement. Responsibilities Conduct comprehensive in-home assessments for new clients and develop individualized service plans. Perform routine client home visits to evaluate care quality, monitor client satisfaction, and update care plans as needs change. Collaborate with caregivers, families, physicians, and office staff to ensure continuity of care. Coach, mentor, and support caregivers to promote exceptional client outcomes. Independently manage a weekly schedule of client visits and complete accurate, timely documentation. Build and maintain strong relationships with clients, families, healthcare professionals, and community referral partners. Represent Assisting Hands at networking events and community outreach activities. Participate in weekly Care Management meetings and interdisciplinary care planning. Assist with caregiver orientation, training, and ongoing education. Participate in the Manager-on-Duty on-call and holiday rotation. Provide occasional hands-on client care during urgent staffing situations when necessary. Qualifications Licensed Practical Nurse (LPN), Licensed Social Worker (LSW), or equivalent healthcare experience required. Minimum of five years of experience in home care, skilled nursing, assisted living, rehabilitation, or a combination of healthcare settings. Previous supervisory or leadership experience preferred. Excellent communication and interpersonal skills with the ability to build trust and lasting relationships. Strong organizational, time management, and problem-solving abilities. Comfortable working independently while managing multiple priorities. Basic computer proficiency, including Microsoft Office. Valid driver's license with an acceptable driving record. Ability and willingness to travel throughout the Greater Cincinnati and Northern Kentucky service area. Passion for serving seniors and supporting and educating caregivers.
