Job Description

Responsibilities:
  • Provide case management services to patients, ensuring their needs are met and their care is coordinated effectively via phone from our Solon, OH office.
  • Utilize knowledge of physiology and patient care to assess and monitor patients' conditions.
  • Collaborate with healthcare professionals to develop and implement individualized care plans.
  • Conduct discharge planning to ensure smooth transitions for patients leaving the hospital.
  • Provide critical care support to patients in need.
  • Maintain accurate and up-to-date patient records.
Qualifications:
  • Must be a certified Medical Assistant or STNA with a valid license.
  • Previous experience in managed care or case management is preferred.
  • Strong knowledge of physiology and patient care practices.
  • Excellent communication and interpersonal skills.
  • Proficient in computers and EMR knowledge.
  • Familiarity with inhale medications and their administration.
