Job Description

Transform Youth and Family Counseling is seeking a compliance-forward, highly organized clinician to join our team in the newly created role of Site Manager for our Grants Pass location. This leadership position combines direct clinical care with a robust administrative, human resources, and facility management role. Operating on-site at our Grants Pass location, the Site Manager leads daily facility operations, supports staff onboarding and compliance, and oversees quality assurance for the building. A central function of this role is serving as the site's administrative operational lead, including managing non-clinical employee performance and performance improvement processes. The Site Manager works collaboratively with Clinical Supervisors while maintaining a clear boundary between administrative oversight and clinical supervision. This is a full-time, salaried exempt position requiring at least 32 hours per week. Reports To:
Associate Clinical Director and Practice Manager Weekly Schedule and Hours Breakdown 32-40 hours total on average
Direct Billable Client Contact:
12-16 hours per week
Client Documentation:
4 hours dedicated to clinical caseload
HR, Operations and Site Management:
12-20 hours dedicated to administrative leadership, staff support, chart audits, facility management, and required Leadership and Staff Meetings Key ResponsibilitiesHuman Resources and Personnel Management Serve as the primary administrative supervisor for employees assigned to the Grants Pass site, with responsibility for addressing attendance, scheduling, policy compliance, workplace conduct, and other non-clinical performance matters. Facilitate new employee onboarding at the Grants Pass site, including building tours, administrative setup, and paperwork completion before transitioning credentialing responsibilities to the Practice Manager. Track, assign, and manage regulatory and compliance-related HR trainings for site personnel. Oversee daily staffing logistics, including managing call-outs, office assignments, staff scheduling, EHR schedule updates, and maintaining an accurate master roster. Organize and administer Performance Improvement Plans (PIPs), disciplinary actions, and other administrative personnel processes in partnership with Clinical Supervisors to promote operational accountability while protecting the clinical supervisory relationship. Complete company-sponsored HR, leadership, and foundational clinical supervision coursework as part of ongoing professional development. Daily Facility and Administrative Operations Manage local vendor relationships, office supply procurement, and site bank cards for the Grants Pass location. Develop and maintain a thorough understanding of practice policies and procedures to serve as an on-site resource for staff. Maintain building safety and regulatory compliance by organizing emergency drills, coordinating fire inspections, and facilitating required safety trainings in accordance with Certificate of Approval and Oregon Health Authority requirements. Support site communication by maintaining shared calendars and staff communication spaces. Receive, document, triage, and coordinate resolution of staff and client complaints and grievances, escalating clinical, ethical, legal, safety, discrimination/harassment, and other high-risk matters to appropriate leadership. Quality Assurance and Compliance Perform regular chart audits for staff assigned to the Grants Pass office. Complete required ROADS updates at least every seven days. Monitor clinical hour requirements for therapists and support accurate administrative tracking. Clinical Duties Provide individual, family, and group therapy to youth ages 6-24. Complete clinical documentation in accordance with practice policies and applicable requirements.
Qualifications and Requirements Education and Credentials:
Master's-level clinician who is a registered QMHP, LPC, LMFT, LCSW, or board-registered associate.

Excellent written and verbal communication skills with a strong foundation in establishing and maintaining professional boundaries.
