Remain knowledgeable about program regulations, community support services, licensing requirements, and funding sources. Build and strengthen relationships with program staff, healthcare providers, and community partners to coordinate care and services. Maintain tracking systems to identify individuals requiring comprehensive treatment planning. Conduct monthly and quarterly coordination visits with internal and external providers. Provide direct supervision and support to Care Coordinators and Resource Coordinators. Facilitate routine multidisciplinary treatment team meetings and collaborative planning sessions. Monitor and report quality outcomes, statistics, and performance measures. Complete weekly and monthly reporting requirements accurately and timely. Conduct systematic reviews of client records to ensure comprehensive care coordination and resource utilization. Develop comprehensive treatment plans for individuals receiving services across multiple programs and settings. Review treatment plans to ensure compliance with program, funding, and regulatory requirements. Collaborate with internal departments and external partners to promote quality care and positive outcomes.
