Responsibilities:
Assume full responsibility for the day-to-day operations and human resource management of the Case Management department Oversee the interdisciplinary plan of care and the discharge planning process to ensure effectiveness and appropriateness of services, with a central focus on census management, patient care outcomes, and key care indicators Serve as a patient and family advocate to ensure services meet the needs of patients and their families Provide guidance, support, and training to Case Managers and staff on caseload management, regulations, policies, operational procedures, and objectives Review departmental operations to maintain a high level of quality consistent with organizational standards Build and maintain relationships with insurance companies, self-insured employers, case management firms, and other healthcare networksQualifications:
Must be qualified to independently complete an assessment within the scope of practice of his/her discipline Active license required if licensure is mandated for the discipline in the state Nursing candidates: Bachelor's degree in Nursing (BSN) with active RN licensure required Other eligible healthcare professionals: Minimum of a bachelor's degree required; graduate degree preferred Minimum of three years of hospital-based Case Management experience that includes Utilization Review and Discharge Planning (acute care, inpatient rehabilitation, or LTACH only — SNF, assisted living, home health, or insurance experience will not be considered unless combined with predominantly hospital-based experience) Minimum of two years of leadership experience at the supervisor level or above Open to licensed Social Worker, Physical Therapist, Occupational Therapist, or RN (licensed RN with BSN or higher is highly preferred) Stable career history required (no frequent job hopping) This is a fully onsite role with no remote or hybrid option A bit about us:- A dynamic leader in inpatient rehabilitation focused on high-quality interdisciplinary care, strong patient outcomes, and effective census and discharge management in a 58-bed facility with approximately 275 team members.
