Position Summary:

Under the supervision of the Health Advocate Manager, the Health Advocate Coordinator provides trauma-informed, youth-centered crisis navigation, housing navigation, life coaching, and service coordination for youth impacted by commercial sexual exploitation, domestic violence, homelessness, and other forms of violence. The Coordinator conducts outreach, assessments, safety planning, individualized goal planning, referrals, advocacy, and ongoing case management. The Coordinator provides services across Dream's clinic sites, Mobile Clinic, community-based programs, and approved crisis-response locations and must be comfortable delivering services in flexible, public, and nontraditional environments. This position maintains timely and accurate documentation in Dream's electronic health record and required funder data systems and works collaboratively with clinical staff, community partners, and other service providers. Duties and Responsibilities:

Participate in Dream's on-call crisis response rotation and respond according to Dream's safety protocols. Meet youth in crisis at safe community locations, hospitals, police stations, clinics, or other approved locations. Complete immediate, individualized safety planning for every crisis response. Connect youth to urgent shelter, legal, medical, transportation, food, clothing, and other stabilization resources. Accompany and advocate for youth interacting with medical providers, law enforcement, shelters, and other systems. Coordinate participant incentives and goal-related flexible funds in accordance with contract and organizational requirements. Prioritize referrals to partners when appropriate. Conduct outreach and help connect referred youth to healing services Coordinate participant transportation, childcare, scheduling, reminders, and other supports that reduce barriers to participation. Documentation and performance Enter complete, accurate, and timely service data into required systems. Participate in case conferences, program quality reviews, required funder meetings, and trainings. Collaborate with medical teams, and program staff, and community partner professionals. Assess youths' needs, develop appropriate action plans based on needs, document and track progress towards each participant's objectives and goals, use data for continuous improvement, and coordinate timely referrals for service and follow through as appropriate. Engage patients in a comprehensive barrier assessment to address the following barriers: psychosocial health, physical health, mental health, educational needs, family support, substance use, DMV/driver license issues, family/support system, employment/training history, counseling needs, parenting support, access to food, clothing and hygiene items, family reunification, access to healthcare services. Support Dream youth clinic staff in providing team-based care for our youth, including but not limited to, collaborative client management and co-facilitation of supportive groups. Other supportive youth services as identified. Carry a client load of 10-15 patients, depending on case management model and acuity. Develop a long-term relationship with the client/patient that engenders trust. Make patient outreach calls based on patient needs, and provide the patient with her/his business cell phone number for ongoing access. Enroll clients in public benefit programs (ie: Medi-Cal, CalFresh) when applicable. Assist with logistics pertaining to medical and social services. Support medical providers and clinical staff with documentation of patient information. Document encounters in the electronic health record (EHR). Help manage participant flow while protecting privacy, confidentiality, choice, and dignity in a public or nontraditional service setting. Travel between local work sites, throughout the East Bay and occasionally across the state/country. Work on occasional weekends.

Competencies:

A bachelor's degree in public health, business administration, public administration, social work, or related field. Related work experience may be substituted. 2 years' experience working in a non-profit organization, or a community clinic preferred. Experience working with marginalized youth populations locally, including communities of color preferred. Cultural congruence with the Dream Youth Clinic patient population. Enjoy working with diverse youth (cultural, linguistic, and gender diversity). Ability to incorporate creativity and forward thinking in working with youth. Trained/willing to receive training in health coaching, positive youth development, and family planning counseling, and youth sex trafficking. Excellent communication skills. Demonstrated ability to work collaboratively and in team settings. Interest/proficiency in working with urban underserved populations, unsheltered youth, involved in the foster care and juvenile justice system, LGBTQ + and gender expansive youth, or affected by commercial sexual exploitation (CSEC). Experience with benefits enrollment / familiarity with Medi-Cal. Willingness to work a flexible schedule. Valid California Driver License with safe driving record. Ability to establish and maintain effective working relationships with individuals and groups, both professional and non-professional, co-workers, management personnel, the public and others. Solid organizational skills including attention to detail and multi-tasking. Clear and effective external and internal, verbal and written, communication skills. Strong critical thinker and problem solver who can successfully build and lead an execution strategy from a long-term vision. Strong computer skills including proficiency in Google Suite and Microsoft Office. Experience providing outreach, navigation, or direct services in mobile, street-based, shelter, school, community, or other nontraditional settings preferred. Ability to remain calm, follow established safety procedures, and communicate clearly when field conditions change. Ability to engage young people respectfully while protecting privacy and confidentiality in public settings. Requirements:

Frequent standing, stair climbing, bending, stooping, lifting (25 lbs. max). Gross and fine manual dexterity. Ability to visually observe staff and clients to understand and interpret non-verbal behavior. Ability to operate standard office machines and equipment including computers, computer software, copy machine, scanner, and keyboards. Ability to work outdoors in changing weather and around noise, crowds, uneven surfaces, and other community-based conditions. Personal transportation preferred. Current participants in Dream Youth Clinic programs are not eligible for employment. Former participants may be considered for employment after a minimum separation period of 12 months from active program participation.
