Join a Mission-Driven Organization All Care Hospice is seeking a compassionate, motivated, and dynamic Hospice Clinical Manager (RN) to lead our clinical team and help provide exceptional end-of-life care to patients and families throughout the communities we serve. This is an excellent opportunity for an experienced nurse leader who is passionate about patient-centered care. Hospice experience is preferred, but we are willing to train the right candidate with strong leadership and home health or related clinical experience.

Schedule:

Monday through Friday with a hybrid work schedule, including four days in our Lynn office and one remote workday each week. As the Hospice Clinical Manager, you will provide clinical and operational leadership, support staff development, ensure regulatory compliance, and foster a positive team culture while making a meaningful impact on patients and families. Essential Duties & Responsibilities Provide day-to-day leadership and oversight of hospice clinical operations to ensure exceptional patient care and efficient workflow. Supervise, mentor, and support clinical staff through coaching, performance evaluations, and professional development opportunities. Monitor staffing levels, caseload assignments, and productivity to support quality care and operational effectiveness. Collaborate with the interdisciplinary team to coordinate patient care and ensure individualized plans of care are implemented and updated. Monitor patient and family outcomes and respond proactively to changing clinical needs. Ensure compliance with Medicare Conditions of Participation, state and federal regulations, accreditation standards, and agency policies. Participate in recruitment, interviewing, hiring, onboarding, and retention of clinical staff. Lead quality improvement initiatives, clinical audits, performance improvement projects, and outcome monitoring activities. Assist with developing and implementing clinical policies, procedures, and best practices. Coordinate staff orientation, education, competency validation, and ongoing clinical training. Promote a positive, collaborative culture that supports employee engagement, accountability, and excellent patient and family experiences. Qualifications Required Active Massachusetts Registered Nurse (RN) license. Minimum of two years of nursing experience in hospice, home health, palliative care, case management, or a related clinical setting. Prior supervisory, leadership, charge nurse, or management experience. Strong organizational, communication, and problem-solving skills. Proficiency with EMR/EHR systems and Microsoft Office applications. Valid Massachusetts driver's license, reliable transportation, and proof of automobile insurance. Preferred BSN degree. Hospice experience and knowledge of Medicare Hospice Conditions of Participation. Experience leading interdisciplinary clinical teams.
