MDS Coordinator - Skilled Nursing Facility (SNF)
Key Responsibilities Coordinate and complete MDS assessments and resident care plans. Collaborate with nursing staff, physicians, residents, families, and interdisciplinary teams. Ensure documentation and assessments meet federal, state, and facility requirements . Participate in interdisciplinary care-plan meetings. Provide nursing care and perform daily nursing duties as needed. Requirements Nursing degree from an accredited college or university. Active California RN or LVN license . Current CPR certification . At least 1 year of verifiable nursing experience in a skilled nursing or long-term care setting preferred. Previous MDS experience preferred. MDS certification preferred. Current California RN license preferred. Strong communication, organization, leadership, and time-management skills. Active
